Cancellation Policy

At Formby Injury & Rehab Clinic, we understand that plans can change.

We appreciate that unforeseen circumstances sometimes mean appointments need to be cancelled or rearranged. Our cancellation policy is designed to be fair to both our patients and our clinicians, while helping us make appointments available to those who need them.

When you book an appointment with us, that appointment time is reserved exclusively for you. Late cancellations often leave insufficient time for us to offer the appointment to another patient.

This Cancellation Policy explains how cancellations are managed, when charges may apply, and the circumstances in which cancellation fees may be waived.

1. Booking an Appointment

When you book an appointment with Formby Injury & Rehab Clinic, you are reserving dedicated clinical time with one of our healthcare professionals.

By confirming your appointment, you acknowledge that you have read and accepted this Cancellation Policy.

2. Cancelling or Rearranging Your Appointment

If you need to cancel or rearrange your appointment, we kindly ask that you give us as much notice as possible.

Providing sufficient notice allows us the opportunity to offer your appointment to another patient waiting for treatment.

3. Late Cancellation Charges

Appointments cancelled at any time on the calendar day before the scheduled appointment, or on the day of the appointment itself, will be charged at 100% of the appointment fee.

For example:

• Appointment booked for Tuesday at 1:00 pm
• Cancellation made on Monday at 9:00 am

Although this is more than 24 hours before the appointment, the cancellation was made on the calendar day immediately before the appointment. As such, the full appointment fee will apply.

This policy applies regardless of the number of hours between the cancellation and the appointment.

4. Failure to Attend

If you fail to attend your appointment without notifying the clinic beforehand, the full appointment fee will be charged.

Repeated missed appointments may result in future bookings requiring payment in advance or, where appropriate, the clinic declining to offer further appointments.

5. Extenuating Circumstances

We understand that genuine emergencies and unexpected situations can occur.

If exceptional circumstances prevent you from attending your appointment, you are welcome to contact the clinic to explain your situation.

Examples may include, but are not limited to:

• Sudden illness
• Family emergency
• Bereavement
• Caring responsibilities
• Significant travel disruption
• Other unforeseen exceptional circumstances

Each request will be considered individually.

Any decision to waive or refund a cancellation fee is made entirely at the discretion of Formby Injury & Rehab Clinic. We are under no obligation to waive a cancellation charge, but we will always aim to act reasonably and fairly when considering individual circumstances.

6. Why We Have a Cancellation Policy

Our cancellation policy is not intended to penalise patients.

It exists to:

• Protect valuable clinical appointment time
• Allow other patients access to appointments
• Support the sustainability of our healthcare services
• Respect the time of our clinicians and staff
• Reduce unnecessary waiting times

Late cancellations often mean an appointment cannot be filled at short notice, resulting in lost clinical time that could otherwise have been offered to another patient.

7. Payment of Cancellation Fees

Cancellation fees may be collected using the payment method held on file or may be invoiced for payment.

Outstanding cancellation fees may need to be settled before further appointments can be booked.

8. Changes to This Policy

We may update this Cancellation Policy periodically to reflect:

• Changes to clinic procedures
• Improvements to our services
• Changes in applicable laws or regulations

The latest version will always be available on our website.
